Orthodontically Driven Osteogenesis 2024

Improving orthodontic treatment by reducing side effects and expanding clinical limits has been a long-standing goal. Although corticotomy is an established technique, confusion about its terminology, application, and benefits has limited its acceptance. While the procedure has achieved wide horizontal diffusion among pioneers, vertical diffusion within orthodontic communities remains low, highlighting the need for clearer education and dedicated resources.

Despite strong and consistent evidence supporting its safety and effectiveness, orthodontic attention has shifted toward 3D digital planning and clear aligners. Terminology has evolved toward concepts such as corticotomy-facilitated orthodontics, accelerated osteogenic orthodontics, and especially orthodontically driven osteogenesis (ODO), emphasizing bone formation rather than regeneration.

ODO, particularly when combined with bone grafting, enables basal bone expansion, reduces the need for premolar extractions, and strengthens the periodontium, decreasing the risk of gingival recession. This approach introduces tissue engineering into orthodontics and offers faster, safer treatment, while providing a less invasive alternative to orthognathic surgery.

From an innovation perspective, ODO has shown strong horizontal diffusion but limited vertical adoption, largely due to poor dissemination, lack of commercial promotion, and misunderstanding of its benefits. In this context, 3D digital planning may facilitate broader acceptance by clearly demonstrating when orthodontic movement exceeds the original bony envelope.
